Medical Records Release Policy
Medical records will be released if we have a HIPAA-compliant signed medical-release for that patient, signed by the patient or her legal guardian. Records will be mailed within 10 business days. Records that are needed sooner than that may be picked up by the patient or faxed to a medical provider’s office. We will require proof of identification for pick-ups.
Medical records will not be sent or accepted via fax. All records must be sent to us via U.S. Mail.
We will release the records for the previous 12 months and any abnormal results at no charge to the patient. Additional pages will be released at a cost of 75¢ per page, plus postage. This is in accordance with New York State laws.
If you are unable to pay the fee, contact the office manager at 315-472-6935. You may be asked to complete a hardship form in order for us to make a decision.
All medical record requests must be made in compliance with state and federal regulations. Please use our HIPAA compliant form to request your records. You may also use this form to have records transferred to us from another provider. Click here to download.
